What determines the chance to regain function post prostate surgery?

Erectile function depends on numerous physical and psychological factors. Thus, the likelihood that a man will regain function post prostate surgery depends on many factors, including preoperative function. Many patients have diminished erectile function or no erectile function before surgery and are at highest risk of losing erections with prostate surgery. Patients with satisfactory erections before prostate cancer surgery often have risk factors that can interfere with function after prostate surgery, including hypertension, diabetes, obesity, atherosclerosis, history of smoking, anxiety, and others. The only technical factor relating to erectile function that is under the control of the surgeon is the sparing of the peri-prostatic neurovascular bundles. Anatomical preservation of the bundles does not denote functional preservation of erections.
